Sefton Carers Centre Hospital Discharge Support Service offers unpaid carers and family members living in Sefton support when caring for someone who is due to or has recently been discharged from hospital.

We can advise and support the carer when navigating the discharge processes, also helping liaise with various health and social care professionals and services such as hospital discharge teams, ward nurses, Specialist nurses, Social workers, occupational therapists, GP's and other community-based health and social care services. 

We will ensure that the carers voice is heard throughout the discharge process, pushing for carers to be considered equal partners when dealing with hospital discharge teams. 

We are aiming for people to access the service ideally whilst still in hospital, or after recently being discharged from hospital. Both professional and self-referrals will be accepted by contacting Sefton Carers Centre via email, or via the carers centre switchboard and website.
